This website is operated by Luna Health, Inc., the study sponsor, to help people find out whether they may qualify for a research study being run at one or more clinical research sites. It does not provide medical care.

What the screener asks. Your age, diabetes type, insulin and CGM use, overnight glucose, a few health conditions and medications, whether you have a care partner, and your ZIP code. The questions do not ask for your name, your medical records, or your insurance; the optional contact form at the end asks for a first name, a phone number and a preferred call time.

Your answers. Your answers stay in your browser while you answer, and closing the page clears them. We do not store your individual answers. They are sent only if you choose to send your details (below), apart from the anonymous counts described under Tracking. Your ZIP code is used on your device only, to find the nearest research site, and is never sent to anyone.

If you choose to send your details. If you tick the consent box and submit the contact form, your first name, phone number, preferred call time, a summary of your screener answers and, if you came from an ad, which ad, are emailed to the research site you are matched with or choose, so its study team can call you. Luna's server passes them on to the site and keeps no copy.

Tracking. This site does not use advertising pixels, analytics services or cookies. We do not sell your information or share it for advertising. We keep anonymous daily counts, by ad, of how many people reach each step of the screener; of each result (likely or possibly eligible, and which research site they were matched with); and, when the screener ends early, of the reason it ended (for example, "uses an insulin pump"). The counts contain nothing else: no other answers, no IP address, nothing that identifies you. Cloudflare hosts this site and delivers the email. Like any host, it sees your IP address to deliver pages; we do not keep it.

Your choices. Sending your details is optional. To ask a research site to delete them, call that site (phone numbers are on the About the study page). For any privacy question, email privacy@lunadiabetes.com.
